Facts in brief:
A worker was transporting a mobile elevating work platform (MEWP) on an 8-wheeler Isuzu flatbed truck. When unloading the MEWP from the truck, one of the wheels slipped from the edge of the truck causing the unit to lift and tip with the victim still in the basket. He was catapulted from the basket striking the ground and was seriously injured.
