It appeared that a worker had made contact with a cable that according to external contractors was 13KV. The worker was admitted to hospital and treated.

An investigation was completed and the report showed that an existing cable was not repaired correctly, resulting in bare conductors being exposed. This cable has then made contact with the worker during the maintenance of the ozone generators.

Repairs were made on the unit and documentation was provided upon completion of the job. Subsequently, a decision was made to remove the aging ozone unit and install a new ozone unit.

WorkSafe Energy Safety made contact with the companies involved with the incident. A detailed project plan for the installation of a new ozone generator, along with Health and Safety documentation was provided.
