If someone has been seriously injured, become seriously ill, or died as a result of work – phone us on 0800 030 040 straight away. We have staff available to respond to these 24/7.

Notifications made through our online form won't be monitored between 12pm on Tuesday 24 December 2024 and 8.30am on Monday 6 January 2025.

The Fire Service determined the point of fire origin to be in the electrical plug between a hot plate and the extension cord that it was plugged into. The hot plate used in the smoker was found near a door in an area of greater damage believed to be the area of origin.
Energy Safety examined the information and photos supplied by the Fire Service. Energy Safety was not able to determine that the cause of the fire was from the plug, because a plug will be constructed from fire inhibiting materials, therefore it would require a suitable fire loading around it for a failure to sustain an ignition source. It would be more likely that a failure with the hotplate or the cooking process involving the hotplate may have been the cause. From the information available Energy Safety has not be able to determine the exact cause of the fire.
