Use the following to make an application for a Grade A or Grade B bore manager certificate or to renew your existing certificate.

For a new bore manager certificate or to renew an expired certificate complete the following from.

A-Grade Bore Manager applications are in relation to geothermal work involving drilling where the bore is, or is to be, 150 metres or more in depth.

B-Grade Bore Manager applications are in relation to geothermal work involving drilling where the bore is, or is to be, up to 150 metres in depth.

Please provide the following with the completed application form:

• 1 x passport size photo of yourself 
• Updated CV
• Copy of your current well control certificate
