Facts in brief:
A scaffolder removing scaffolding around a residential dwelling was electrocuted when a scaffolding pole he was holding while dismantling the scaffold made contact with 33kV overhead power lines.

The defendant company had sought and obtained a close approach consent to operate in the proximity to the lines, but did not comply with the requirement of the consent to send the same workers who had received a safety briefing and installed the scaffolding to disassemble the structure.

The victim had both of his arms amputated, and lost the use of his legs due to the electrocution injuries.
