Notifications
If someone has been seriously injured, become seriously ill, or died as a result of work – phone us on 0800 030 040 straight away. We have staff available to respond to these 24/7.

Notifications made through our online form won't be monitored between 12pm on Tuesday 24 December 2024 and 8.30am on Monday 6 January 2025.
If you’re not sure if you need to notify us, use our online notification system and we’ll respond to you after 6 January 2025.

The defendant failed to have:
- Ensured an adequate system was in place to capture, communicate and comply with the requirements of close approach consents and Field Service Provider safety briefings.
- Ensured the scaffold was dismantled in accordance with the requirements of the close approach consent and the Field Service Provider’s safety briefing.
The defendant company had sought and obtained a close approach consent to operate in the proximity to the lines, but did not comply with the requirement of the consent to send the same workers who had received a safety briefing and installed the scaffolding to disassemble the structure.
The victim had both of his arms amputated, and lost the use of his legs due to the electrocution injuries.
