Energy Safety determined that the machine involved had been the subject of a safety recall and that the owner had been previously sent a letter from the manufacture advising them of the recall. However, the owner did not respond to that letter and continued using the washing machine. Responsibility for the fire has to be shared between the manufacturer and the owner of the machine who was aware of the safety issue, but continued to use the machine without taking any action.
