Notifications  
If someone has been seriously injured, become seriously ill, or died as a result of work – phone us on 0800 030 040 straight away. We have staff available to respond to these 24/7.

Notifications made through our online form won't be monitored between 12pm on Tuesday 24 December 2024 and 8.30am on Monday 6 January 2025.
If you’re not sure if you need to notify us, use our online notification system and we’ll respond to you after 6 January 2025.

General enquiries made by phone or email after 12pm on Tuesday 24 December will be responded to from Monday 6 January 2025. This does not apply to notifications made by phone on 0800 030 040.

The Health and Safety at Work (Hazardous Substances) Regulations 2017, Regulation 13.34(6), requires a Person Conducting a Business or Undertaking (PCBU) to notify WorkSafe of a temporary storage site of a 6.1A, 6.1B or 6.1C substance associated with the imminent start of a pest control operation or pesticide application task under certain circumstances.
You must notify WorkSafe at least 24 hours before commissioning the place where the substance will be stored.
The form below is for notification under Regulation 13.34(6) by a PCBU to WorkSafe. Please complete the form and include "Temporary Storage" in the subject line of the email, and send the completed notification form to healthsafety.notification@worksafe.govt.nz.
If you do not meet the criteria above (Regulation 13.34(6) for temporary storage) you may need to consider establishing a Hazardous Substance Location and notifying WorkSafe, this can be achieved by using our online notification form.
