A quarry worker suffered fatal injuries after becoming entangled in rotating machinery at a South Canterbury quarry. The operator was working alone at the time.
Circumstances
The exact time and circumstances that led to this fatality are unknown. He was only discovered after a neighbour noticed the quarry machines were still operating much later in the day than normal and went to investigate.

WorkSafe is investigating this incident.
We wish to bring to the sector’s attention the need to ensure all machinery is properly guarded, and fences restricting access to machinery are properly secured. Perimeter guarding does not negate the need for individual guards on dangerous moving parts.
Guidance
Operators should guard machinery to meet AS/NZS 4024, as well as other relevant standards. Worksafe New Zealand provides extensive guidance material relating to machine guarding and in May 2014 it produced a publication titled: Safe Use of Machinery.
